1. Each of the forms should be saved (you could try {right-Click} of the mouse) on your desktop.
2. Use an Adobe Reader or Adobe professional to open the form (*pdf file)
3. Use the “Typewrite tool” in Adobe to type in the information in the form.
OR – Simply print the form and write the information
4. You can print or save the completed form on your desktop.
5. Simply fax or email the form once complete.
